1) What is the purpose of collecting this information online in advance of my appointment?

By receiving this information well in advance of your appointment, Dr. Lee can actually view your history and concerns prior to your appointment and make any special arrangements and/or preparations that may allow your visit to go more smoothly.
We understand that your time is truly valuable. With your registration information already in our system, your check-in process can be extremely efficient and you are typically back to see Dr. Lee in less than 5 minutes.
2) Is my information secure?
Yes. The New Patient Registration Form is secured by a third party certification authority. All information submitted through the New Patient Registration Form is protected health information and will not be used or distributed in any other way than to treat your skin condition.
3) Some of the questions asked on the form seem unrelated to my medical concerns. Why is this information necessary?
We deeply respect your time and only want to gather information necessary to appropriately treat your concerns. Information regarding race, skin type, occupation and various other categories can often play a critical role in determining a patient’s risk for certain skin conditions (i.e. skin cancer). Additionally, the Centers for Medicaid and Medicare Services (CMS) requires reporting of certain demographic and personal information as part of their meaningful use initiative of electronic health records to improve things like access to health care.
4) How many times will I need to complete the New Patient Registration Form?
Once. The only other time we will ask you to complete this form is if three years have elapsed since your last visit.
5) Should I complete the New Patient Registration Form online well in advance of my appointment?
Yes. Every patient who has completed the form in advance is happy they did and feels they got the most out of the time spent at their appointment.
